PLEASE NOTE: You must provide your tax exempt certificate when placing an order.
Order Form
Attention Members of the Global Hospital Exchange (GXH): If you are currently processing EDI orders through GHX, no additional action will be required. Mammotome is now a vendor on GHX and can easily process your order.
Return Policy: Need to return an unused item? Capital equipment and disposable products that are unused and saleable must be returned within 90 days of the invoice date. All other saleable products must be returned at least 12 months prior to the expiration date. All saleable returns will receive 85% credit (15% restocking fee). Please contact your Mammotome Customer Service Representative to schedule a merchandise pick up and answer any questions you may have regarding our return policy.
