AVAILABLE NOW, Mammotome offers you three simple and convenient ways to place an order.

Order Placement – Choose the option that works best for you. 

    1. PHONE: Please contact your Mammotome Customer Service Representative at 1-877-9-A-MAMMO between the hours of 8:30am and 6:30pm EST. Shipping details and an order confirmation number will be provided to you upon completion of your order.
    2. FAX: Send your order via fax to your Mammotome Customer Service Representative at 1-888-260-6362.
    3. EMAIL: Send your order via email to your Mammotome Customer Service Representative at customerservice@mammotome.com.

PLEASE NOTE: You must provide your tax exempt certificate when placing an order.

Order Form

Attention Members of the Global Hospital Exchange (GXH): If you are currently processing EDI orders through GHX, no additional action will be required. Mammotome is now a vendor on GHX and can easily process your order.

Return Policy: Need to return an unused item? Capital equipment and disposable products that are unused and saleable must be returned within 90 days of the invoice date. All other saleable products must be returned at least 12 months prior to the expiration date. All saleable returns will receive 85% credit (15% restocking fee).  Please contact your Mammotome Customer Service Representative to schedule a merchandise pick up and answer any questions you may have regarding our return policy. All product returns must be sent to: Kuehne+Nagel, Inc., ATTN: Devicor Medical Products, Inc., 2580 West Walnut Avenue, Rialto, CA 92376 (Telephone 1-909-574-2300).
